Title 5 Building Reviews – Form C & Form S

A Title 5 Building Review is required before applying for a building permit for most types of construction—including additions, renovations, finishing rooms, or new structures. These reviews help ensure that the property’s septic or sewer system can support any changes in use or layout.

Concord uses Form C for septic-connected properties and Form S for sewer-connected properties. Room count, design flow, and system location are all part of the review process.

Title 5 Inspections

A Title 5 Inspection is required under Massachusetts regulations when a property is sold, transferred, or significantly renovated. The inspection ensures the existing septic system is functioning properly and complies with 310 CMR 15.000 – Title 5.

Submission Requirements:

  • Title 5 Inspection Reports must be submitted online only through PermitEyes.
  • The inspector listed on the report must be permitted in the Town of Concord for the report to be accepted.
    • A list of permitted Title 5 inspectors is available upon request.

Replacing, Repairing, or Installing a Septic System (Form A & Form B)

All septic system work—including new installations, repairs, and full system replacements—must comply with Massachusetts Title 5 and Concord Board of Health regulations.

  • A Form A – Soil Test Application is required when designing a new system or when soil conditions need to be evaluated.

  • A Form B – Disposal System Construction Permit is required for all system work, including repairs, replacements, and new installations, regardless of whether soil testing is needed.

All plans must be reviewed and approved by the Health Division before work begins. Installers must be licensed by the Town of Concord.

Private Wells
well

Private well owners are responsible for maintaining the safety and quality of their own drinking water. Regular testing is essential to ensure the water is free of contaminants and safe for household use.

In Concord, private wells must comply with the Concord Board of Health’s Minimum Sanitation Standards for Private and Semi-Public Water Supplies, which follow guidance from the U.S. Environmental Protection Agency (EPA) and the Massachusetts Department of Environmental Protection (MassDEP).
